It's 99% zbrush with a few PS highlights & details painted on top.

As for the title, it's more a comment on hyper real fx grotesquery being 'the new beautiful' in movies than it is an explicit ref to Frankenstien... but maybe Frankie puts on the bolts and stitches in make-up, just like any other actor
